Анотація

МЕТА. Узагальнити сучасні наукові дані щодо актуальних підходів до діагностики, фенотипування та лікування гастроезофагеальної рефлюксної хвороби з урахуванням сучасних принципів персоналізованої медицини.

МАТЕРІАЛИ ТА МЕТОДИ. Проведено наративний огляд сучасної наукової літератури та міжнародних клінічних рекомендацій, присвячених питанням діагностики й лікування ГЕРХ. Пошук літературних джерел здійснювали в базах даних PubMed/MEDLINE, Scopus, Web of Science Core Collection та Google Scholar. Аналіз включав клінічні настанови, консенсусні документи, систематичні огляди, метааналізи та оригінальні дослідження із застосуванням порівняльного, аналітичного та узагальнювального підходів.

РЕЗУЛЬТАТИ. Проаналізовано сучасні концепції діагностики ГЕРХ, зокрема роль ендоскопічних та функціональних методів дослідження (амбулаторного pH-моніторингу, pH-імпедансного моніторингу, манометрії) у підтвердженні патологічного рефлюксу та визначенні механізмів формування симптомів. Особливу увагу приділено сучасному фенотипуванню захворювання з виділенням ерозивної рефлюксної хвороби, неерозивної рефлюксної хвороби, рефлюксної гіперчутливості та функціональної печії. Охарактеризовано сучасні підходи до лікування ГЕРХ, включаючи модифікацію способу життя, оптимізацію терапії інгібіторами протонної помпи, застосування калій-конкурентних блокаторів кислотної секреції, алгінатів та інтервенційних методів лікування. Визначено необхідність індивідуалізованого вибору терапевтичної стратегії залежно від фенотипу захворювання, механізму виникнення симптомів і наявності ускладнень.

ВИСНОВКИ. Сучасне ведення пацієнтів із гастроезофагеальною рефлюксною хворобою ґрунтується на переході від симптом-орієнтованої моделі до персоналізованого фенотип-орієнтованого підходу. Об’єктивна верифікація патологічного рефлюксу та визначення механізмів формування симптомів є ключовими для вибору ефективної терапії, покращення контролю захворювання та якості життя пацієнтів.
